Waiver Form and Assumption of Risk and Release of Liability Agreement.

 

I, the client participating with instruction from The Natalie Berkeley Studio, assume all responsibility for and all risk if damage or injury that may occur as a result of my own actions, inaction, or negligence, or that of others as a client of Natalie Berkeley. In consideration of and as part of payment for the right to participate as a client of The Natalie Berkeley Studio I will hold harmless and release and discharge all rights and claims for damages that I may have or that may hereafter accrue to me against The Natalie Berkeley Studio for any and all injuries suffered by me caused in whole or in part by my failure to faithfully follow instructions or by any physical impairment of mine, not fully disclosed to The Natalie Berkeley Studio in writing. The terms hereof shall serve as release, indemnification, and assumption frisk for my heirs, executors, and administrators, and for all members of my family. I understand that The Natalie Berkeley Studio offers exercise and informational classes, but does not offer medical treatment, services, or a replacement thereof. The Natalie Berkeley Studio does not provide diagnosis of any physical ailments or conditions. I, the client participating with instruction from The Natalie Berkeley Studio, acknowledge that I may participate in a class when I am pregnant, have recently given birth, or am recovering from injury or surgery if I have been cleared medically. I, the client participating with instruction from The Natalie Berkeley Studio, acknowledge, that I am physically and mentally healthy and do not suffer from any disability that would prevent or limit my participation. If I am pregnant, have given birth, or am recovering from an injury, I understand that it is my responsibility to consult with a physician prior to and regarding my participation. If I am the legal guardian of a client that is under the age of 18, I consent to the above listed Waiver Form and Assumption of Risk and Release of Liability Agreement for my minor. Effective as of June 1st, 2020.
